About the service
The social prescribing service is provided by Central Camden PCN to support people whose health and wellbeing may be affected by practical, social or lifestyle challenges. Social prescribing recognises that many non medical factors, such as housing, finances or social connection, play an important role in how people feel. Link Workers help patients find practical support and connect with services in their local community.
How can social prescribing help?
Social prescribing link workers can support patients with a range of non medical needs, including:
- Housing or money worries
- Work, employment or benefits support
- Confidence, motivation and wellbeing
- Finding and accessing local groups and community services
The focus of the service is on helping patients identify what matters to them and supporting practical steps toward positive change.
Who is the service for?
The social prescribing link worker service is available to:
- Adults aged 18 and over
- Patients registered with one of the following GP practices only:
- Ampthill & Regents Park Practice
- Bloomsbury Surgery
- Brunswick Medical Centre
- Kings Cross Surgery
- Somers Town Medical Centre
- People who feel ready to take action and make changes to improve their wellbeing
Who is the service not suitable for?
The social prescribing link worker service is not suitable for crisis situations or therapy needs. If you need urgent mental health support, please contact your GP practice or appropriate crisis services.
How does the service work?
- Your GP refers you to the Social Prescribing service
- A link worker contacts you to arrange a telephone or a face-to-face appointment
- You and the link worker agree a simple, personalised action plan based on your goals and needs
The link worker will support you to access relevant services and community support.
When and where are clinics held?
Social prescribing clinics are held on the following days at the following GP practices:
- Monday: Bloomsbury Surgery, Brunswick Medical Centre
- Tuesday: Brunswick Medical Centre
- Wednesday: Brunswick Medical Centre (AM), Bloomsbury Surgery (PM), Kings Cross Surgery
- Thursday: Kings Cross Surgery
- Friday: Bloomsbury Surgery
How do I access the service?
