New Patient Registration
We accept new patients who live in our practice area (the WC1 area) as well as NHS Staff that live or work locally. We accept out of area patients under the out of area scheme, but please note, we cannot offer home visits to patients registered with us under this scheme.
How to Register
Complete a New Patient Registration Form
If you live within the boundary area, please complete our new patient registration
To register with us please complete our online registration form or download a GMS1 Registration Form and bring it into the practice, or you can obtain a paper copy of the form from reception. It can take up to five days for us to fully process your registration. Once completed, your medical records will be transferred to us and a new medical card will be sent to your home address. You do not need to inform your existing practice that you are moving, although it may be courteous to do so.
Your Named GP
All patients at Bloomsbury Surgery are free to see any GP of their choice but each patient has a named accountable GP on their clinical record. If you wish to find out who your named GP is then please ask at reception. This named GP has overall responsibility for you but please feel free to request an appointment with any doctor or our Nurse Practitioner who may be available.
