What is the health check clinic?
The health check clinic is a new service provided by Central Camden PCN to support patients in staying on top of their health and wellbeing. The clinic offers a structured health check and follow up discussion to help identify potential health concerns early and provide personalised advice and ongoing support.
What does the health check clinic involve?
The service is delivered in two steps:
Step 1 - health check (location may vary between practices)
During the Health Check appointment, patients will receive:
- A blood pressure check
- Blood tests
- Height and weight measurements
Step 2 - follow up discussion (location may vary between practices)
This follow up appointment allows time to:
- Review test results
- Discuss individual health needs
- Create or update a personalised care plan
Appointments for the follow up discussion last around 30 minutes

How are appointments booked?
Patients do not need to book directly. The PCN team will contact eligible patients by phone to arrange appointments.
