Diabetes Clinic
What is the diabetes clinic?
The diabetes clinic provides dedicated time, expert support and access to the latest tools and treatments to help patients manage their diabetes effectively. The clinic focuses on personalised care, ensuring each appointment is tailored to individual needs and health goals.
Who is the clinic for?
The diabetes clinic supports patients living with diabetes who may benefit from additional review, advice or specialist input to help improve control and overall health. Eligible patients will be contacted about an appointment by the PCN and appointments will take place at your registered practice.
Who delivers the clinic?
Patients attending the diabetes clinic will be supported by a multidisciplinary clinical team, which may include:
- A GP
- A Pharmacist
- A Physician Associate
Patients will have at least one appointment with the clinical team. Initial appointments may last up to one hour.
What can I expect at my appointment?
During an appointment, patients may receive support with:
- Reviewing blood sugar levels and understanding diabetes control
- Health checks, such as blood pressure and weight, where appropriate
- Personalised advice on medicines, monitoring and healthy living
- Discussion of specialist treatment options or technology, where suitable
The clinic aims to provide time, clarity and practical support to help patients feel confident in managing their diabetes.
