Nutritional Therapy Programme

 

What is the nutritional therapy programme?

The PCN Nutritional Therapy Programme supports patients to improve their nutrition and manage health conditions through tailored nutritional advice and guidance. The programme helps patients build confidence around food choices and develop healthier, balanced eating habits that support long term health and wellbeing.

 

What does the programme offer?

Patients referred into the programme receive personalised support from a nutritional therapist to help them:

  • Understand how diet affects their health
  • Make realistic and sustainable changes to eating habits
  • Improve health outcomes and overall wellbeing through better nutrition

Support is tailored to each individual’s needs, preferences and health goals.

 

Who can access the programme?

The PCN Nutritional Therapy Programme is available to adults aged 18 and over who are registered with a GP practice in Central Camden PCN and who:

  • Want to lose or gain weight
  • Have high cholesterol or high blood pressure
  • Live with IBS or IBD
  • Experience chronic fatigue
  • Are diabetic or pre diabetic
 

How does the programme work?

  • The programme is delivered by Central Camden PCN
  • Nutritional advice is tailored to individual needs
  • Sessions may take place online or in person
  • Referrals are made by clinicians only
 

Programme structure

Once referred, patients are supported through a structured Nutritional Therapy Programme which includes:

  • An initial one to one consultation - face to face or online, approximately 1 hour
  • A first follow up consultation - telephone appointment, approximately 45 minutes
  • A second follow up consultation - telephone appointment, approximately 15 minutes
  • A final one to one consultation - face to face or online, approximately 1 hour

This structure allows time to assess individual needs, review progress, and provide ongoing support to help patients make long lasting changes to their nutrition and lifestyle.

Patients who think they may benefit from the programme should speak to their GP practice, who can advise on suitability and referral.
